A 17-year-old woman was referred due to mandibular prognathism and facial asymmetry. Concave profile with full cusp Class III molar relationship on the right side was observed. Anterior and posterior cross bite on left side, midline discrepancy, reduced maxillary incisal exposure and peg lateralis were also presented. Dental arch was decompensated by maxillary second premolar extraction, constricted transpalatal arch and inter-arch elastics during presurgical orthodontic treatment which was followed by mandibular stabilization splint. Functional occlusion was obtained after the orthognathic surgery and post-surgical orthodontic treatment. Prosthodontic treatment of the peg lateralis was done after the orthodontic treatment.
